✏️ Source write-back (highlight + comment box)

Given a source byte-range (M18-I01) + a thought, edit the source file in the
Barthes-proven format: wrap the passage ==…== line-by-line (no crossing
newlines), insert a > [!note] callout with the thought immediately after
the passage, and set frontmatter type: "[[reading-comment]]" idempotently.
Atomic write; reversible plain markdown. M11 renders the result.

Acceptance

A multi-line passage is wrapped line-by-line; the > [!note] box follows it;
type is set once (idempotent on re-run); reopening renders highlight + comment
